You don’t think of yourself as particularly anxious or a bad sleeper. You may not have anything particularly stressful / traumatic happening in your life when it happens. And yet here you are again ~ wide awake at 3:47am, heart running a little too fast, brain suddenly very interested in a conversation you had last week. Like me and many women I know, you may have noticed an increase in these 3am wake-ups since you entered your 30s.

You fall asleep fine ~ that's the confusing part. But staying asleep has become more consistently challenging; somewhere between 3 and 5am, like clockwork, you're awake.

Here's what no one tells you: this wake-up isn’t random, and it's not just stress. It's physiology. Your body is doing something specific, for specific reasons ~ and once you understand what it’s communicating you can actually start to support it.

Reason one: your blood sugar crashed (and this isn’t just about willpower).

You ate dinner at six. Maybe it was on the lighter side, maybe you thought you were "being good," and didn’t eat a lot of carbs. Maybe you ate on the go and stopped eating before you were satiated. By 3am you've gone nine hours without food, and your blood sugar has dropped low enough that your body hits the emergency button. It releases cortisol and adrenaline to pull stored sugar back into your blood and keep your brain fueled.

When we under eat or when we are experiencing blood sugar instability our liver is not able to store enough glycogen to sustain us overnight. So the body increases these ‘stress’ hormones as a survival mechanism, thinking that we are running out of the glucose we need to survive.

Those two hormones do their job, but they also happen to be the exact hormones that wake you up ~ often with a pounding heart or a wave of anxiety that has nothing to do with your actual life. Or another signal is you may wake up just feeling very awake, no pounding heart or anxiety, but like your brain is fully on immediately rather than a slow, cozy wake up that you experience from a full deep sleep.

Quick test: if you wake at 3am and a few bites of food (think some cheese & honey for protein, carbs and fat) put you back to sleep, blood sugar was your culprit. You can also try the same for a bed time snack to see if that helps sustain you.

Reason two: your nervous system never actually clocked out.

If you spend your days in go-mode ~ pushing, achieving, holding it all together ~ your nervous system gets calibrated to stay switched on. You lie down, but your body is stuck in sympathetic and doesn’t go into relax mode with you. It idles in fight-or-flight all night, which keeps your sleep light and means any small dip (see reason one) is enough to surface you. (Are you starting to see how these things are interconnected?)

Your nervous system runs on minerals. Potassium, magnesium, sodium, calcium ~ these are the macro-mineral raw materials it uses to regulate the stress response and put the brakes on. When you're depleted (and if you've been running fast for years, you are), the body’s breaks are not working very well. The stress response can fire at the smallest provocation, including a normal overnight blood sugar dip.

Clients I often see with this pattern show low or very high levels of the calming minerals calcium and magnesium. This indicates that their body doesn’t have the raw materials needed to actually put the brakes on and shift into the parasympathetic nervous system properly.

Reason three: the luteal-phase tax (and the perimenopause one).

In the two weeks before your period, progesterone raises your metabolic rate and your core temperature ~ which means your body needs more fuel, as much as 300 extra calories a day. If you don’t follow your hunger cues to eat more during this time, or have a pattern of under eating, you're now running a deficit, and the blood sugar dips in reason one get louder.

Progesterone, which increases in the luteal phase, does something else, too. Its calming metabolite acts like your body's own built-in sedative ~ it helps settle your nervous system at night. As you move through your late 30s and 40s, progesterone starts to decline and fluctuate, so this can play a part in losing that nighttime calm. Add rising, erratic estrogen, which tells your body to make more histamine and slows the enzyme that clears it ~ and we have less calming hormone and more wakefulness hormone. Histamine is a wakefulness signal (it's the exact thing an antihistamine blunts when it makes you drowsy). More histamine on board means lighter sleep that breaks more easily and a more hypervigilant system.

None of this is you failing at sleep. It's a body that's under-fueled, under-resourced, and navigating a hormonal shift with an empty tank.

Reason four: the histamine factor

This needs it’s own category even though it is quite intertwined with reason three (and all the others, honestly). Estrogen and histamine are synergistic, meaning surges of estrogen can cause an increase in histamine in the gut. When elimination slows down ~ think constipation, slower motility, hemorrhoids, estrogen your body already used and sent out for removal gets reabsorbed in the gut and recirculates instead of leaving. More estrogen back in circulation means more histamine released right alongside it. If you notice an increase in seasonal allergies, this is a sign that your histamine bucket is overloading. Histamine intolerance and MCAS are very nuanced and interrelated topics that I’m not going to get into fully here, but speaking as someone recovering from severe histamine intolerance, women often experience more histamine issues as their estrogen starts to fluctuate, their bodies become more depleted from decades of stress, child bearing, etc. Histamine alone can keep one in that wired and tired state, feeling kind of itchy, unsettled, with a mind running wild.

Quick test: for a few nights pull high-histamine foods out of dinner ~ wine, aged cheese, cured meats, leftovers, soy sauce, anything fermented ~ and see if the 3am wake-up softens. If it does, histamine's in the mix.

how to stop waking at 3am

Eat enough ~ general recommendations for women are hitting 2,000 calories per day with 3 consistent meals and snacks as needed throughout the day. For sleep support you want to ensure you are getting enough food at dinner, and also ensure you are eating more in your luteal phase (generally the two weeks before your period). This looks like animal-based protein for essential amino acids, and nutrients, healthy saturated fats (not pufas!), and plenty of whole-food carbs. Do not shy away from carbs - they are quick, easily digestible energy sources for us and many women are slow oxidizers meaning we need that quicker energy. And if you wake at 3am, keep something simple by the bed: a spoon of honey, a piece of cheese, with a pinch of good salt does the trick ~ a little glucose to hold your blood sugar through the night and a few minerals to settle the system. Note the bed time snack is not a long-term solution, but it will help you get a feel for your hunger cues and help you fall back asleep if blood sugar is the culprit.

Feed your nervous system the raw materials it needs. Our bodies need sufficient nutrients every day to thrive. How much of what you need, and how depleted you actually are, is specific to you ~ but the foundation is real, mineral-rich food from whole-food animal based diets with sufficient carbs and fiber: potassium and magnesium from plants and animal foods, calcium from good quality dairy, with enough sodium to support tired adrenals.

Down-regulate before bed for real. You cannot drop into deep sleep from a sympathetic state. Give your body a direct parasympathetic signal: restorative yoga, knitting, humming, slow exhales that run longer than your inhales, coherent breathing recordings, yoga nidra. These are vagus nerve stimulation which tells your body it's safe to rest.

And a wildcard most people miss: your neck positioning. Your vagus nerve runs right through it. Chronic tension and a poorly supported neck at night can keep that nerve from doing its down-regulating job. A better pillow setup (I look Coop or Sutera) and a little gentle neck release before bed is a small thing that can make a big difference.

The challenging thing about the 3am wake-up experience is it is a confluence of variables that change every month based on your cycle, eating habits, stress levels, and environment. So you might notice that sometimes blood sugar supports but other times you need a lower histamine diet. And this can be really confusing to track.
